October 13, 2017, Brooklyn, New York – Community Health Initiative, a federally qualified health center that serves the residents of Coney Island since 2016 has been awarded the prestigious designation as a Primary Care Medical Home (PCMH) Level III by the National Committee for Quality Assurance (NCQA). NCQA is the leader in health care accreditation dedicated to improving health care quality. CHI’s PCMH Level III designation, the highest achievable level, is valid for three years.
As a level 3 patient centered medical home, CHI is recognized as a health center that practice a model of care that puts patients at the forefront of care, builds a better relationship between providers and patients, and reduce health care cost by meeting six key criteria. These criteria include 1) Ensure access to care; 2) Use data to conduct population management; 3) Care Management; 4). Support self-care process 5). Track referrals and follow-up; and 6) Implement continuous quality improvement processes.
The esteemed designation will also avail CHI the opportunity to receive higher reimbursement and incentive awards from health insurance payors for being a high quality and effective provider. The monetary rewards will be reinvested to further enhance the quality of care and access for CHI patients. Some of the enhancements will include offering telehealth to expand hours and services and facilitate care management for patients with chronic conditions.
