solution (CDoc Online) to provide comprehensive remote care, inlcuding Chronic Care Management (CCM) and Transitional Care Management (TCM).

  • Sign up:
    Once you sign up from https://www.cdoconline.com, please call us 732-800-0020 to order devices. We will ship the devices to your clinic.

  • Setup
    Setup appointment with patient for enrollment and setup. Our customer support will walk you thru the whole process.

  • Review Data
    Review patient data and have a MA or nurse to call your patient for their status. At least 20 minitues per patient per month is required to bill 99457. Document it in care management system.

  • Get Paid
    Bill to insurance plan and get paid. You can download the billing report from our system to bill Medicare for CPT 99453, 99457, 99458 and 99454.

You keep the Medicare payment for CPT 99453, 99457 and 99458. For each month, we only charge you for CPT 99454 minus co-pay, to cover our hardware, cellular services, hosting, and customer support.

CPT code 99453: "Remote monitoring of physiologic parameter(s) (e.g, weight, blood pressure, pulse oximety, respiratory flow rate), initial; set-up and patient education on use of equipment."
What to know: CPT 99453 offers reimbursement for the work associated with onboarding a new patient onto a RPM service, setting up the equipment and educating the patient on using the equipment. The average national Medicare payment for these service is $19.46.
CPT code 99454: "Device(s) supply with daily recording(s) or programmed alert(s) transmission, each 30 days."
What to know: CPT 99454 offers reimbursement for providing the patient with a RPM device for a 30-day period. Note that 99454 can be billed each 30 days. The average national Medicare payment for these services is $64.15.
CPT code 99457: "Remote physiologic monitoring treatment management services, 20 minutes or more of clinical staff/physician/other qualified healthcare professional time in a calendar month requiring interactive communication with the patient/caregiver during the month."
What to know: Under this new code, CMS will reimburse for clinical staff time that contributes toward monitoring and interactive communication which includes phone, text and email. The average national Medicare payment for these serivces is $51.43 (non-facility) and $32.44 (facility).
CPT code 99458: "Remote physiologic monitoring treatment management services, clinical staff/physician/other qualified health care professional time in a calendar month requiring interactive communication with the patient/caregiver during the month; additional 20 minutes."

Integrated Care Management

Chronic Care Management (CCM) + Transitional Care Management (TCM)

The CCM + TCM service is extensive, including structured recording of patient health information, an electronic care plan addressing all health issues, accessing care management services, managing care transitions, and coordinating and sharing patient information with practitioners and providers outside the practice.
 
CMS requires the use of certified EHR technology to satisfy some of the CCM scope of service elements. In furnishing these aspects of the CCM service, CMS requires the use of a version of certified EHR that is acceptable under the EHR Incentive Programs as of December 31st of the calendar year preceding each Medicare PFS payment year (referred to as "CCM certified technology").

Why does it matter to me?

According to the Center for Medicare & Medicaid Service (CMS), CCM is recognized as one of the critical components of primary care that contributes to better health and care for individuals, as well as reduced spending. Beginning January 1, 2015, Medicare pays separately under the Medicare Physician Fee Schedule (PFS) under American Medical Association Current Procedural Terminology (CPT) code 99490, for non-face-to-face care coordination services furnished to Medicare beneficiaries with multiple chronic conditions.
